In order to initiate the development of a LifeCare Plan, we will need to receive the following:

1.  Admission & discharge summaries, operative reports, consultations and social service reports from all hospitalizations

2.  Outpatient records and reports with regard to diagnoses and treatment, including expert reports secured to identify physical and/or cognitive status and prognosis

3.  School records (if pertinent)

4.  Bills incurred as a result of the injury and/or an itemized medical bill summary

5.  Name, address and phone number of client (or guardian)  -  It is our practice to meet with the patient to assess his/her needs.

6.  The date by which you must have the completed LifeCare Plan

7.  A signed copy of our standard contract for services (Please e-mail us using the link below and check off  the "Send Contract" option.)

8.  A retainer check in the amount of $1,500 made payable and sent to:
